Dear St. Patrick Catholic School and Clover Patch Preschool Families,
We are excited to offer final registration materials in a digital format again this year. Instructions and steps for completion and submission are below.
Fillable forms allow for easy electronic signature and return. A computer is recommended. The business office will mail invoices for the 2022-23 school year. Payment is due by the first day of school. -

Final Registration Instructions

Select from these options for completing and returning items required in the yellow section below.
Two ways to fill out packets:- ELECTRONICALLY: Download, SAVE, fill out electronically, and SAVE file to your computer (preferred).
- MANUALLY: Print single sided and fill out by hand.
Three ways to return packets: Note: if you plan to return by mail or drop off, please print single sided.
- EMAIL the saved digital files (or scanned paper copies) to [email protected]
- MAIL paper forms to St. Patrick School, 615 Washington St., Cedar Falls, IA 50613.
- DROP OFF paper forms in the locked mailbox inside the glass door at 615 Washington on August 4 or 5 from 8 AM-3:30 PM.

Steps for Final Registration and Payment

Every family will complete Steps 1-3 and 5. Step 4 is for new students only.
STEP 1: Determine what pages you need to fill out in the "Family Packet." Leave those that do not apply to your family blank and fill out those you need with the link and instructions below this list.Pages to choose from:- Circle of Care/Day Care Enrollment Form (PS-Gr 8 families using before or after school care)
- Bus Intent Form (if riding the bus and have not yet been in touch with the office)
- Non-Public Transportation Reimbursement Form (Kindergarten-Gr. 8 non-bus riders living more than 2 miles from St. Patrick School) Choose the appropriate form:
- Residents of Cedar Falls or school districts other than Waterloo
- Residents of Waterloo School District only
- Free and Reduced Lunch Application-only to be filled out by families that meet the eligibilty requirements.
- If your family does not meet the eligibilty requirements, or has received a direct certification letter, please leave the form blank.
- For those who do quailify, please note:
- Meals are not going to be FREE to all students in SY2022-23. The USDA is transitioning back to the National School Lunch Program (NSLP) and School Breakfast Program (SBP). Students that are eligible and approved for free or reduced price meals will continue to receive meals at no charge or at a reduced meal price.
- Many school benefits are based on our free and reduced lunch count. This includes federal funding for remedial reading and math services through the Title I program, grants including the Universal Services Fund for technology, and the Archdiocesan Educational Grant program.
- Read this letter from Mr. Wiebers for more information. If you feel you quality, we appreciate your time filling out this confidential application so that we have the appropriate funding for our students. Instructions.
Link and instructions: To avoid loss of data, click "Family Packet" and use the download arrow (upper right corner) to first save the file to your computer, adding your family name to the file name (example: Jones_Family Packet). Open and fill out your SAVED copy. Then SAVE your file again after completing. If you choose to print and fill out by hand, print one-sided.
STEP 2: Download and fill out the appropriate "Student Packet" for each child.
Use the tip above to download and save, this time adding the student first and last name (example: Jones Sarah_Kindergarten Student Packet) or print one-sided.- Clover Patch Student Packet
- Kindergarten Student Packet (insert a photo file in the box on the Who's Who sheet)
- Grade 1-4 Student Packet
- Grade 5-8 Student Packet
- 7th grade students will need an updated immunization record on file with the required meningococcal shot (return a copy of the printout from your doctor)
- Students interested in athletics need to fill out the athletic agreement and the concussion form.
- If needed, download the Sports Physical Form required annually for athletes
STEP 3: Complete Express Enrollment in PowerSchool (all families) by August 5. Information under the Home Language Survey tab must be filled out for all new students. For liability reasons, Enrollment Express/permissions must be completed annually for your child to attend school.
STEP 4: Submit these items for NEW STUDENTS (email, mail, or drop off).
- Physical/immunization form for preschool-grade 8 (return a copy of the printout from your doctor)
- Dental screening form for kindergartners only (take this form to your dentist or use one of theirs)
- Federal Ethnicity Form
- Copy of baptismal certificate for K-8 (if applicable)
- Copy of birth certificate (optional)
- Complete the information under the Home Language Survey in Enrollment Express
STEP 5: Submit payment.An invoice will be mailed to you. Payment is due upon receipt.- Mail a check to St. Patrick School, 615 Washington St., Cedar Falls, IA 50613.
- Drop off a check to the lock box inside the glass door at 615 Washington on Aug. 4 or 5 from 8 AM-3:30 PM.
- Pay online via the Charger Webstore (it's easy but a fee will apply).
